General Membership Application
General Membership is $50,_ Residents, Students and Retirees $10
To be eligible for membership, you must be a Physician, Dentist, Resident, Student or Fellow, in good standing. Membership includes an invitation to – our Annual General Membership Meeting held in the month of August.
In order to process your Membership Application please read BAMS & LAMS Bylaws and Code of Ethics and then complete the form below and if application is accepted you will be prompted to submit your Annual Dues.
